On July 1, 2011, we filed on behalf of the Medical Provider, Dr. Albert Skocik, a Petition for Review of Utilization Review Determination for the reasonableness and necessity of all chiropractic treatment, prescriptions, and referrals provided by Albert Skocik, D.C. from March 28, 2011, and into the future. Jane L. McBride, D.C. had issued a Utilization Review Determination concluding the treatment was not reasonable.

Theresa Noble (her name is changed to protect her privacy) was injured at Hershey Medical Center while working for Sharpsmart exchanging and installing needle boxes. Her 40 hour per week job involved installing needle boxes, removing them from the wall and transporting them on large rolling bins. Her quota is 60 boxes per hour and this includes servicing and exchanging the boxes.

Employee BT, a resident of  York County, suffered a work-related repetitive stress injury to both hands and wrists while employed by Harley Davidson as an electrician. He hired us as his workers’ compensation lawyers and we filed a Pennsylvania (Pa) Workers’ Compensation Claim Petition on behalf of the injured worker.

Employee XT, a resident of New Brighton, Beaver County, suffered a work-related injury in the nature of a right foot fracture while employed by Professional Service Industries Inc. We filed a Claim Petition on requesting payment of medical bills, counsel fees and litigation costs.

The Commonwealth Court has held that a truck driver who receives his dispatch instructions from his home in Pennsylvania, sometimes starts and ends his trips at his Pennsylvania home, and spends a substantial part of his time driving in Pennsylvania, is entitled to Pennsylvania Workers’ Compensation benefits. In Williams v. W.C.A.B. (Pohl Transportation), No. 2422 C.D. (June 25, 2010), Claimant sustained a work injury to his right leg while in Vermont, but received Ohio workers’ compensation wage loss benefits.

The injured worker, a resident of Oberlin, Dauphin County, PA, suffered a work injury to the low back while employed for Pitt Ohio Express in 1994. Recently, he treated with Stuart Hartman, D.O. for the work injury, who prescribed medications filled by Injured Workers’ Pharmacy.

Employee ST, a resident of Camp Hill, Cumberland County, suffered a work injury on March 2, 2009, while employed by GGNSC Camp Hill III, LLP. While performing her duties as a Certified Nursing Assistant, she twisted her knee while helping to move a patient from an ambulance.

The injured worker, a resident of Middletown, Dauphin County, PA, suffered a work injury on May 23, 2008, while employed by Colonial Park Care Center as a Staff Development Coordinator and Nurse Aid Instructor while helping a student move a patient into bed.  Claimant felt a “give” in the center of her back and pain radiating down to her lower back. Claimant was diagnosed with a low back strain. Claimant was prescribed medications and underwent physical therapy for her lower back but still experienced significant pain. Claimant began treating with Dr. Albert Skocik, DC, for low back manipulation, electrical stimulation, therapeutic exercises and massage therapy, until the end of March 2009.

The Claimant, a resident of Waynesboro, Franklin County, Pennsylvania, suffered a work injury while employed by Woodward Camps Inc as a Skateboard Counselor/Instructor. He was performing a skateboarding trick and fell wrong and his knee dislocated, resulting in immediate swelling and pain and the claimant could not walk after the injury.

The Claimant, a resident of Lewistown, Dauphin County, filed a Review Petition on March 14, 1996, alleging that the Employer has failed to pay the Claimant’s workers’ compensation benefits on a weekly basis. The Claimant was paid on a weekly basis prior to his work injury. After the Claimant was placed on compensation, he received it on a weekly basis. After the Employer, ABF, acquired Carolina Freight Carriers, the Claimant received it on a biweekly basis. The Claimant received some of the biweekly checks two to three weeks late, causing his bills to be paid late.
